The extract is from the novel "The Remains of the Day" by Kazuo Ishiguro. It is found at the end of the novel. Stevens visitis Miss Kenton in the town she lives in, in the end of his journey.
In this extract, Miss Kenton reveals some of her thoughts about her life, and confesses how she feels about Mr Stevens. She says in the third paragraph of the extract: "What a terrible mistake I've made with my life." [...] For instance, I get to thinking about a life I might have had with you, Mr Stevens." In response to this, Mr Stevens for the first time admits openly to himself that he actually has strong feelings for Miss Kenton, which he expresses in the fourth paragraph. "- why should I not admit it? -at the moment, my heart was breaking." This thought only occurs in Mr Stevens mind and towards Miss Kenton he remains rather distanced.
It is worth mentioning that throughout the extract, as well as throughout the whole novel, Mr Stevens and Miss Kenton always address each other by their last names. This contributes to the maintainance of distance between the two characters.
In the first paragraph, where Miss Kenton speaks about her husband, she refers to him as "my husband" or "him", whereas she refers to her daughter by her name. Hence it gives the impression that she is distancing herself from her husband.
